Desired functionality is to add a new action to /monitor that will generate an ephemeral embed with everyone who is currently being monitored, their Discord ID, who added them, and when. The reason will not be provided, with the embed explaining the philosophy, and that the person who performed the monitoring should be contacted instead.

While I think the "out of sight, out of mind" mantra stated in the docs is valid and correct, ultimately, it is not feasible for server moderators to not be able to review the list of people who are being monitored. Otherwise, without technical knowledge and access, they cannot easily remove people from the list.
